CASE MANAGER
Lakeport, CA | $25.99–$32.28/hour
We are seeking a compassionate and highly organized Case Manager to provide housing-related services to families, at-risk adults, and individuals experiencing homelessness through the Bringing Families Home and Home Safe grant programs. This position provides hands‑on case management, housing assistance, benefits advocacy, and coordination with internal departments and community agencies.
Key Responsibilities
- Conduct client intake assessments and develop individualized service plans.
- Provide ongoing case management, housing assistance, rental support, and benefits advocacy.
- Help clients locate appropriate housing and work with landlords to secure placements.
- Assist clients with housing applications and supportive services.
- Develop SMART goals to help clients work toward stability and self‑sufficiency.
- Coordinate services with internal departments, community organizations, and external agencies.
- Maintain accurate case files, progress notes, and program documentation.
- Track client progress and ensure compliance with grant guidelines and budgets.
- Prepare and submit required grant reports by established deadlines.
- Maintain strict confidentiality, particularly with sensitive cases.
- Participate in team meetings, training, and professional development.
Qualifications
- Associate’s degree in Social Work, Behavioral Science, Social Science, or related field;
OR high school diploma with at least two years of relevant case management, housing services, or social services experience. - Experience providing direct services to clients in a social services environment.
- Strong organizational, communication, documentation, and problem‑solving skills.
- Ability to handle confidential and sensitive information professionally.
- Proficiency with Microsoft Office and electronic data/case management systems.
- Valid driver’s license and satisfactory driving record.
- Ability to complete a 40-hour Domestic Violence training course within six months of hire.
- Ability to successfully complete required background check, fingerprinting, and drug screening. Employer observes federal controlled‑substance standards, including cannabis.
Compensation
$25.99–$32.28 per hour
, depending on applicable experience, skills, and education.
