* Copy of Transcript (Must be Official Transcripts)

* Credential Copy (Copy of 30 Day Substitute Teaching Permit or Valid California Teaching Credential (Multiple Subject, Single Subject, Special Education, etc)

* Fingerprint Clearance (Copy of "Clearing House" live scan form (if applicable) - May be completed at a later date)

* TB Screening Result (Copy of current TB test results or Risk Assessment Questionnaire (last 4 years) - If you do not have this, VCOE will provide you with further instructions)

* Test Results/Materials (Copy of CBEST Passing Scores)

Transcripts:
Copies of official transcripts from all colleges/universities attended, including undergraduate coursework up to highest level of education must be submitted. All official / copies of transcripts can be submitted by one of the following:
1) Emailed directly to VCOE from your college / university
2) Mailed via U.S. mail to Ventura County Office of Education Attn:
Substitutes 5189 Verdugo Way Camarillo, CA 93012.
* If utilizing the email option, please have transcripts emailed to: pmartin2
* Any sealed hard copy official transcripts you receive directly from your college/university can be provided to the HR department as well. VCOE will examine your transcripts to determine if the college(s) / university (universities) is /are regionally accredited.
