Shift Type: Tuesday-Saturday, 8:30 AM - 5:00 PM (Day shift) or Sunday-Thursday, 8:30 AM - 5:00 PM (Day shift); on-call duties and floating between units required
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities Conduct thorough patient assessments and document findings accurately in Epic
Develop and implement individualized plans of care focused on recovery and function
Provide gait training, ADL training, and use of assistive devices to maximize independence
Deliver rehabilitation modalities such as cryotherapy and moist heat when appropriate
Perform wheelchair assessments, prosthetic training, and safe transfer techniques including Hoyer lift use
Reassess patients regularly, document progress, and coordinate discharge planning
Collaborate with interdisciplinary teams including respiratory, pharmacy, radiology, and social services to optimize outcomes
Float between acute care and acute rehab units as needed and respond to on-call requests
Qualifications Minimum 1 year of acute care hospital experience (explicitly noted)
Active state physical therapy license required (pending licenses are not accepted)
BLS certification preferred
Experience charting in Epic is required
Comfortable floating between like units and responding to on-call duties
Trauma Level II experience preferred but not required
Knowledge & Skills Epic charting and accurate documentation practices
Patient assessment, plan of care development, and reassessments
Gait training, ADL training, and use of adaptive devices
Prosthetic training and wheelchair assessment skills
Hands-on experience with modalities such as cryotherapy and moist heat
Safe lifting and transfer techniques including Hoyer lift and sit-to-stand devices
Familiarity with billing populations:
Medicare A, Medicare B, Medicaid, and commercial
Ability to work with adult and geriatric patient populations and neurological, cardiopulmonary, oncology, and orthopedic cases
