Job Summary As a Licensed Vocational Nurse (LVN) with Buena Vista Home Health, you will follow the patient’s plan of care and provide treatment under the supervision of a Registered Nurse. This is a home health nursing position focused on quality care and patient outcomes.

Responsibilities Implement plan of care initiated by the RNProvide accurate and timely documentation

Collect assessment data and report observations to RN and physician

Coordinate home health services with the interdisciplinary team

Use equipment and supplies effectively

Participate in professional growth and development

Requirements

Valid California LVN License Current CPR certification2+ years of direct patient care experience

Licensed driver with insured, reliable vehicle
