Description
- Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
- Utilizes equipment and technology skillfully and is technically competent in nursing procedures
- Oversees and/or participates in safe medication handling and practices
- Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
- Acts as a patient advocate. Provides support, information and direct care
- Triages phone calls to identify patient/family needs and notifies Admission Services of referrals
- Oversees and delegates assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
- Participates in weekend, holiday, on call and/or evening/night rotations
- Maintains appropriate licenses and adheres to continuing education obligations
- Ability to cover different areas of hospice care to assure organizational needs are met
- Provides medical care to hospice patients in an inpatient setting
- Completes assessments, administers medications, and manages plan of care for patients during their shift
- Coordinates discharge planning with family and medical provider
- Provides medical care to patients in their home environment based on triaged needs
- Utilizes nursing assessment and symptom management skills to assist with urgent after hours needs
- Educates patient/caregiver on plan of care in coordination with the on-call medical provider

- Develops plan of care alongside an Interdisciplinary Team in line with the patient/family needs and goals
- Coordinates all aspects of home hospice care including symptom management, spiritual, psychosocial, financial, and bereavement needs
- Works closely with the Interdisciplinary Team to assure patient/family goals are met
Education and Work Experience Requirements:
- Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
- BSN from an accredited college preferred
- CHPN certification expected within two (2) years of employment
- Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA and other federal, state and local laws
