Coverage Needed
- May 1, 2026 - November 30, 2026
- Schedule:
Monday through Friday, 8:00 AM to 5:00 PM - No call required
Clinical Details
- Provide comprehensive primary care to PACE participants
- Manage chronic and acute medical conditions
- Participate in interdisciplinary team meetings
- Coordinate care with nursing, social work, and therapy staff
- Document care in the electronic medical record
- Patient Volume:
Smaller patient panels due to high patient complexity - May include clinic visits, home visits, nursing home coverage, or phone/in basket coverage
- Support Staff: APP and Physician coverage available
- EMR: EPIC
Requirements
- Active MA License
- Board Certified
- Flu Vaccination
- Must Have Current/Most Recent Direct Supervisor Reference
