Overview

The Pavilion Rehab & Nursing Center is a Five Star, Joint Commission accredited short-term rehab, long term, and skilled nursing care community located in Hyannis. We are dedicated to our residents and team members. We are seeking highly motivated and qualified nurses to join our nursing department.

Sign on bonuses: RN up to $4000; LPN up to $3000 (24+ hours).

• Assess the health care needs of the resident, make a nursing diagnosis, transition this, through collaboration with the nursing team, into an interdisciplinary plan of care utilizing all resources available. Communicate this plan to all care providers to ensure continuity of care for residents.
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff and community.
• Assess the effects of medications and treatments, monitor these effects, and provide feedback to the interdisciplinary team and medical managers to ensure the resident receives appropriate treatment.
• Use of the Electronic Health Record— PCC preferred.
• Lead and direct the nursing staff in the safe, effective delivery of care by providing a structure. Educate residents regarding their healthcare issues and care plan strategies.
• Maintain knowledge of infection control, fire and disaster policies, and other facility, state, and federal policies and practices to provide a safe environment for residents, community and staff.
• Protect resident rights and treat residents, staff and the community with respect and dignity. Maintain confidentiality and manage facility property and information responsibly.

• RN salary range based on experience $41.00-$53.95
• LPN salary range based on experience $36.70-$49.45
• Weekday shift differential:
  Nights $5.00 per hour
• Weekend shift differential:
  Nights $7.00 per hour

RN Per Diem rate is $47.00 per hour with shift differentials above. LPN Per Diem rate is $42.00 per hour with shift differentials above.
