In-Home NP Clinician PT (Job Code: 530764)

Signify Health, part of CVS Health®, is hiring part‑time board‑certified nurse practitioners to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. In this role, you will conduct pre‑scheduled in‑home visits with health plan members, primarily traveling locally within a designated metro area. Primary coverage areas include, but are not limited to, Springfield, MA.

• Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month.
• Execute a strong visit completion rate, pacing 35+ completed visits per month.
• Maintain availability Monday–Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.
• Conduct in‑home Health Evaluations and follow‑up visits (approximately 45‑minute pre‑scheduled visits) that include medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan.
• Maintain all required supplies and associated diagnostic equipment related to diagnostic and preventive service (DPS) offerings.
• Participate in video visits as needed and maintain training and compliance with video visit requirements.
• Accommodate field observations and/or ride‑along experiences as requested.

• Board‑certified nurse practitioner with active state licenses in coverage areas (FNP or AGNP certification preferred).
• Board certification required; multi‑state licenses preferred.
• Experience with Signify Health In‑home Health Evaluations, in‑home visits, or a similar format preferred.
• Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s).
• Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics.
• Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies (including iPad, medical supplies, and scale).
• Be flexible to travel locally and within licensed states, as needed (up to 25%).
• Maintain training and complete all diagnostic and preventive service (DPS) products as directed.
• Maintain supplies and associated diagnostic equipment related to DPS offerings.
• Maintain multi‑state licenses (preferred).
• Maintain board certification.
• No requirement to prescribe medicine, order lab tests, or alter current treatment regimens.
