We are currently seeking an Assessment Specialist to join our Ohio Home Care program. As an Assessment Specialist, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
- Completes visits to assess for eligibility for specified program and conducts safety assessments, in compliance with Care Star and department guidelines.
- Documents all applicant and case-related contacts and interventions in applicants record per agency and Care Star policy, including standards of content and timeliness.
- Processes specific Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and Care Star policy.
- Provides assistance with community resource referrals as appropriate.
- Identifies and reports high-risk situations as required per agency and Care Star policy.
- Adhere to the Care Star Rule in performance of job responsibilities.
- Understand and comply with Care Star Policies and Procedures.
- Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follow the Acceptable Use Policy while using any information systems owned or controlled by Care Star, Inc.
Minimum Qualifications
- Active License as RN, LSW, LISW or LPN.
- 12 months experience in home and community-based services within the past 5 years.
- Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
- Adheres to the Care Star Rule in performance of job responsibilities.
- Understands and complies with Care Star Policies and Procedures.
