• Complete MDS 3.0 assessments (PPS, OBRA, admissions, quarterly, annual)
• Serve as an MDS Nurse / MDS Coordinator ensuring accuracy and timeliness
• Maximize reimbursement under PDPM
• Participate in care plan meetings and interdisciplinary collaboration
• Monitor and improve Quality Measures (QMs) and Five-Star outcomes
• Ensure compliance with CMS guidelines and RAI Manual
• Submit assessments timely through iQIES
• Support survey readiness and audits

• Active RN or LPN license (required)
• Experience as an MDS Nurse, MDS Coordinator, or MDS RN/LPN preferred
• Strong knowledge of MDS 3.0, PDPM, and Medicare A
• Detail-oriented, organized, and reliable
• Team-focused RN or LPN who communicates well with staff
