T-LPN

Job Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a Long-Term Care setting under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for administering medications, monitoring resident conditions, documenting care, and ensuring the highest level of patient safety and comfort.

Key Responsibilities:

• Administer medications, treatments, and injections as prescribed by physicians.
• Monitor residents' health status and report any changes in condition to the RN or healthcare provider.
• Perform routine nursing procedures, including wound care, catheter care, and specimen collection.
• Assess and document vital signs, pain levels, and resident responses to treatment.
• Maintain accurate and timely patient records in accordance with facility policies.
• Assist residents with activities of daily living (ADLs) as needed.
• Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals to develop and implement care plans.
• Educate residents and family members regarding treatments, medications, and health management.
• Ensure compliance with infection control standards, safety regulations, and facility policies.
• Respond appropriately to emergencies and changes in resident conditions.
