LVN ​/ LPN – LTC​/SNF

LVN/LPN Opportunities in Long-Term Care

Provide skilled nursing care in long-term care or skilled nursing facilities as an LVN/LPN. Deliver compassionate patient care while adhering to established medical protocols and facility policies.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient health status and document observations in medical records.
• Assist with patient assessments and report changes to healthcare team.
• Educate patients and their families about care plans and medication usage.
• Maintain compliance with health and safety regulations and facility protocols.
• Collect and validate documentation, including state or federal IDs and vaccination records.

Qualifications:

• Current LVN or LPN license in good standing.
• Proven experience in LTC or SNF settings preferred.
• Ability to wear a photo  comply with facility policies.
• Must have completed COVID-19 vaccination or hold valid religious/medical exemption.
• Strong communication and interpersonal skills.
• Flexibility to work scheduled shifts and adapt to facility needs.
