LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide nursing care to patients in a long-term care or skilled nursing facility setting. Ensure patient safety, document clinical information, and support overall patient well-being through skilled nursing interventions.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report any changes in condition.
• Document patient care activities accurately and timely.
• Collaborate with interdisciplinary team to develop and update care plans.
• Maintain compliance with facility policies and regulatory requirements.
• Assist with patient mobility, hygiene, and comfort needs.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF environments preferred.
• Ability to follow detailed instructions and protocols.
• Strong communication and interpersonal skills.
• Demonstrate professionalism and adherence to safety standards.
• BLS certification required; additional certifications a plus.
