LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN/LPN)

Provide compassionate and skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure residents’ needs are met with professionalism, accuracy, and adherence to facility protocols.

Responsibilities:

• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents’ health status and document changes or concerns promptly.
• Collaborate with healthcare team members to develop and update care plans.
• Maintain a safe and clean environment, adhering to infection control standards.
• Educate residents and their families on health management and care procedures.
• Comply with all regulatory and facility requirements, including identification and safety protocols.

Qualifications:

• Hold an active LVN or LPN license in good standing.
• Demonstrate prior experience in LTC or SNF settings.
• Possess current CPR and BLS certifications.
• Have proof of COVID-19 vaccination or valid religious/medical exemption.
• Exhibit strong communication and interpersonal skills.
