LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN)

Seeking a licensed vocational nurse (LVN) or licensed practical nurse (LPN) for local contract placement in a long-term care/senior nursing facility. This role involves delivering quality patient care in a fast-paced environment.

Responsibilities:

• Provide direct patient care to residents in accordance with physician orders and facility protocols.
• Monitor and document patient vital signs, symptoms, and responses to treatments.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Ensure compliance with infection control, safety standards, and legal regulations.
• Maintain accurate and timely documentation of care delivered.
• Adhere to facility policies regarding identification and PPE requirements, including wearing photo badges at all times and ensuring vaccination compliance.

Qualifications:

• Valid LVN or LPN license issued by the state.
• Proven experience caring for residents in a long-term care or SNF setting.
• Ability to follow detailed instructions and communicate effectively.
• Demonstrate strong assessment skills and clinical judgment.
• Comply with COVID-19 vaccination policies or possess approved exemptions.
• Must bring a valid state or federal  wear a photo badge during shifts.
