LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Seeking a qualified Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for local contract assignment in long-term care or skilled nursing facilities. The role involves providing direct patient care in a supportive healthcare environment.

Responsibilities:

• Administer medication and monitor patient health status following facility protocols.
• Document patient care activities accurately and in a timely manner.
• Collaborate with multidisciplinary teams to develop and implement care plans.
• Ensure compliance with state and federal regulations, including documentation standards.
• Adhere to infection control policies, including COVID-19 vaccination requirements.
• Maintain patient safety and comfort throughout assigned shifts.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Current CPR and BLS certification.
• Demonstrated clinical experience in LTC/SNF settings.
• Ability to follow detailed instructions and communicate effectively.
• Must bring a valid state or federal issued  the facility.
• Must have completed or provide proof of COVID-19 vaccination or a qualified exemption.
