LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Seeking a qualified Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a 14-week contract position providing compassionate patient care in a Long-Term Care / Skilled Nursing Facility setting. The candidate will support clinical operations and ensure high-quality resident care.

Responsibilities:

• Administer medications and treatments accurately following physician orders.
• Monitor patient conditions and document clinical findings in compliance with facility protocols.
• Collaborate with nursing staff and healthcare team to develop and implement care plans.
• Maintain patient safety and comfort by adhering to infection control and safety standards.
• Complete required documentation and reports in a timely manner.
• Follow facility policies regarding staff identification and vaccination compliance.

Qualifications:

• Licensure as an LVN / LPN in the state of practice.
• Proven clinical experience in LTC/SNF environments preferred.
• Successful completion of COVID-19 vaccination or valid exemption documentation.
• Ability to wear a photo  and adhere to facility safety policies.
• Strong communication and interpersonal skills.
