Registered Nurse – LTC​/SNF

Long-Term Care/Skilled Nursing Facility Nurse

Provide skilled nursing care within a long-term care or skilled nursing facility setting. Deliver comprehensive patient assessments and coordinate treatment plans to ensure optimal patient outcomes.

Responsibilities:

• Conduct thorough patient assessments and develop individualized care plans.
• Administer medications and treatments following physician orders and facility protocols.
• Monitor patient conditions and document changes or concerns accurately.
• Collaborate with interdisciplinary teams to ensure cohesive patient care.
• Educate patients and their families on health management and post-discharge care.
• Ensure compliance with all regulatory and safety standards.

Qualifications:

• Valid state registered nurse license required.
• Proven experience working in LTC/SNF environments preferred.
• Complete COVID-19 vaccination or valid exemption documentation required.
• Ability to accurately assess patient needs and adapt care accordingly.
• Strong communication and documentation skills.
• Must present a valid photo  adhere to facility identification policies.
