LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide licensed practical nursing care within a long-term care or skilled nursing facility setting. Ensure patient safety and deliver quality nursing services under supervision, following established protocols and standards.

Responsibilities:

• Administer prescribed medications and treatments accurately.
• Monitor and document patient vital signs and health status.
• Assist with activities of daily living and patient personal care.
• Communicate effectively with healthcare team and patients’ families.
• Follow infection control procedures and maintain a safe environment.
• Complete documentation and reports in compliance with facility policies.

Qualifications:

• Valid LVN or LPN license in the state.
• Demonstrate proficiency in providing patient care in LTC/SNF settings.
• Possess current BLS certification.
• Exhibit strong communication and interpersonal skills.
• Maintain compliance with all COVID-19 vaccination requirements or valid exemptions.
• Ability to work independently and as part of a healthcare team.
