LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide licensed vocational nurse (LVN) or licensed practical nurse (LPN) services in a long-term care or skilled nursing facility setting. Ensure the delivery of compassionate patient care while adhering to strict safety protocols and facility policies.

Responsibilities:

• Administer medication and treatments as prescribed by healthcare providers.
• Monitor patient vital signs and assess their health status regularly.
• Document patient care activities accurately and promptly.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Follow infection control protocols, including COVID-19 vaccination requirements and PPE usage.
• Maintain compliance with all state and federal licensing regulations.

Qualifications:

• Hold a valid LVN or LPN license issued by the state.
• Demonstrate recent experience working in a LTC or SNF environment.
• Possess current CPR and BLS certifications.
• Exhibit strong communication and interpersonal skills.
• Be able to provide a valid photo  comply with facility security requirements.
• Adhere to vaccination policies or have documented exemptions.
