LVN​/LPN LTC​/SNF

Job Summary:

Provide skilled nursing care and support to residents in an LTC/SNF setting as an LVN/LPN. Ensure resident safety, comfort, and well-being through comprehensive clinical practices and effective communication.

Responsibilities:

• Administer medications and treatments in accordance with physician orders and facility protocols.
• Monitor and document residents' health status, including vital signs and changes in condition.
• Assist residents with activities of daily living, ensuring dignity and respect.
• Collaborate with the healthcare team to develop and implement care plans.
• Maintain accurate and detailed electronic health records for each resident.
• Comply with all regulatory requirements, including  policies and vaccination mandates.

Qualifications:

• Valid state licensure as an LVN or LPN.
• Prior experience in LTC/SNF environments preferred.
• Must have completed or be willing to complete COVID-19 vaccination requirements, or hold a documented exemption.
• Strong communication and interpersonal skills.
• Ability to handle residents with compassion and professionalism.
