Job Summary:

The Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) will provide skilled nursing care in long-term care and skilled nursing facility settings. This role involves delivering compassionate patient care, monitoring health status, and supporting the nursing team to ensure optimal patient outcomes.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document patient vital signs, symptoms, and progress.
• Assist with patient assessments and report changes in condition to the healthcare team.
• Provide bedside care, including wound care, hygiene, and mobility assistance.
• Ensure compliance with all safety, infection control, and regulatory guidelines.
• Communicate effectively with patients, families, and team members to coordinate care.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in a long-term care or skilled nursing facility preferred.
• Ability to follow detailed instructions and maintain accurate documentation.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.
• Compliance with COVID-19 vaccination requirements or valid exemption documentation.
