Job Summary:

Provide skilled nursing care to residents in long-term care or skilled nursing facilities. Support patient needs through assessment, medication administration, and care planning to ensure optimal health outcomes.

Responsibilities:

• Administer medications and treatments accurately following physician orders.
• Monitor residents' conditions and document changes in health status.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Educate residents and their families on health management and treatment procedures.
• Maintain compliance with all safety, privacy, and healthcare regulations.

Qualifications:

• Valid state license as an LVN or LPN.
• Demonstrate proficiency in administering medications and performing clinical procedures.
• Possess a minimum of one year of experience in long-term or skilled nursing settings.
• Exhibit strong communication and interpersonal skills.
• Ability to work effectively in a team environment and handle multiple priorities.
