Job Summary:

Providing skilled nursing care in a long-term care or skilled nursing facility, the LVN/LPN will support patient needs through direct clinical care, medication administration, and documentation. This role emphasizes delivering high-quality, compassionate care in a dynamic healthcare environment.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor patient vitals and observe for changes in condition, reporting concerns to the healthcare team.
• Document patient care activities and clinical observations in compliance with regulatory standards.
• Assist with patient assessments and implement care plans as directed by supervising nurses.
• Maintain a safe and clean environment, adhering to infection control protocols.

Qualifications:

• Valid state licensure as an LVN or LPN.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to accurately administer medications and perform clinical procedures.
• Strong documentation skills and attention to detail.
• Must complete or have completed the COVID-19 vaccination or possess valid exemption documentation.
• Excellent communication and interpersonal skills.
