LVN​/LPN - LTC​/SNF

Call for Salary Rate -

Job Summary:

Provide high-quality nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Ensure patient comfort, safety, and adherence to clinical protocols in a fast-paced environment.

Responsibilities:

• Administer medication and monitor residents’ health status.
• Document patient observations and care provided accurately.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Assist with daily living activities and provide emotional support.
• Follow safety protocols, including wearing photo identification badges.
• Comply with vaccination requirements or applicable exemptions as mandated.

Qualifications:

• Valid LVN or LPN license in the state.
• Proven experience in LTC or SNF settings.
• Ability to adhere to infection control and safety protocols.
• Excellent communication and interpersonal skills.
• Demonstrate reliable attendance and professionalism.
• Willingness to comply with COVID-19 vaccination policies or approved exemptions.
