• Provide direct patient care including monitoring vital signs, administering medications, and wound care
• Collaborate with RNs, physicians, and interdisciplinary care teams to develop and implement patient care plans
• Document patient assessments, treatments, and progress accurately in medical records
• Assist with patient admissions, discharges, and transfers in accordance with facility protocols
• Educate patients and families on care instructions, medications, and health management
• Respond promptly to patient needs and changes in condition, escalating concerns as appropriate
• Maintain a safe, clean, and compliant care environment following all regulatory standards
