Short DescriptionA Transitional Case Manager PRN will be able to:
- Provide psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families.
- Formulate the discharge plan with patients, families, and the care team based upon a needs assessment.
- Coordinate appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs.
3 - 5 years health care experience preferred
Education RequirementsRequires a master's degree in social work (MSW) or a licensed RN, BSN preferred
License/Certification RequirementsCurrent NJ SW License, LCSW Preferred.

Current NJ RN License
ACM (American Case Management Association) preferred
CCMC (Commission for Case Manager Certification) preferred
