• Perform PT evaluations and follow‑up visits per physician orders
• Develop and adjust individualized plans of care
• Deliver therapy for gait, balance, mobility, strength, and fall prevention
• Train patients and caregivers on home exercises and safe mobility strategies
• Document per NJ DOH, Medicare, and Bright Star standards
• Collaborate with RNs, OTs, STs, and CHHAs

• NJ Physical Therapy license in good standing
• 1+ year of PT experience (home health preferred)
• CPR/BLS certification
• Reliable transportation within Camden/Cherry Hill
• Strong documentation and patient communication skills
