Physical Therapist

Role Overview

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private homes and senior living communities, utilizing our Geriatric House Calls™ model.

• Perform evaluations, develop personalized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Document care accurately within EMR systems, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual care to enhance access and continuity of care.

• Valid PT license and degree from an accredited program.
• Proficiency in computer and EMR use, current CPR certification.
• Ability to meet physical demands, including lifting, transfers, and extended walking or standing.
