Previous experience as a Clinical Liaison preferred
Experience in a skilled nursing facility required
Working knowledge of skilled nursing facility admissions, referrals, and clinical protocols
Strong communication and relationship‑building skills
Ability to work collaboratively with hospitals, physicians, families, and interdisciplinary teams
