Position Summary
The Licensed Practical Nurse (LPN) provides direct nursing care under the supervision of a Registered Nurse (RN) or physician. LPN supports patient care activities, administers medications, monitors patient conditions, and assists with treatment plans while ensuring adherence to clinical protocols, safety standards, and regulatory requirements. This role promotes a safe, therapeutic, and patient-centered care environment.
- Deliver basic nursing care to patients in accordance with established policies and procedures
- Monitor and record vital signs, symptoms, and responses to treatment
- Administer medications, injections, and treatments as ordered within scope of practice
- Assist RNs or physicians with patient assessments and care planning
- Collect specimens and perform basic diagnostic tests
- Provide wound care, dressing changes, and implement infection control measures
- Maintain accurate, timely documentation in electronic health records (EHR/EMR)
- Educate patients on medications, treatments, and self-care strategies
- Observe and promptly report changes in patient condition to supervising staff
- Maintain a safe, clean, and therapeutic care environment
- Collaborate effectively with interdisciplinary healthcare team members
- Adhere to HIPAA, OSHA, and all applicable state and federal regulations
Required Qualifications
- Graduate of an accredited Practical Nursing Program
- Current, active Licensed Practical Nurse (LPN) license in the state of practice
- Current CPR/BLS certification
- Knowledge of nursing principles, practices, and procedures
- Ability to follow clinical protocols, physician orders, and regulatory standards
- Strong communication, interpersonal, and teamwork skills
- Previous experience in acute care, long-term care, behavioral health, or correctional healthcare
- Proficiency with electronic medical record systems (EMR/EHR)
- IV certification (if applicable and permitted by state regulations)
- Provides a positive and professional representation of CFG Health Systems, LLC.
- Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and prevention
- Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice
- Adheres to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulations
- Participate in ongoing quality improvement activities
- Completes required orientation as directed by the facility
- Follows facility and OSHA safety rules and procedures while on assignment
- Follows facility and CFG Health Systems Occurrence Protocol
- Upholds HIPAA and PREA regulations
- Punctual and dependable for assigned/confirmed shifts
- Maintains security clearance for the assigned facility
