Registered Nurse – LTC​/SNF

Long-Term Care/Skilled Nursing Facility Nurse

Responsible for providing high-quality patient care in a long-term care or skilled nursing facility setting. Ensures compliance with all regulations and maintains a safe, compassionate environment for residents.

Responsibilities:

• Assess patients’ health status and develop individualized care plans.
• Administer medications and treatments accurately and safely.
• Monitor patient conditions and document clinical observations.
• Collaborate with interdisciplinary teams to coordinate care.
• Adhere to facility policies and regulatory guidelines.
• Maintain clear communication with patients and their families.

Qualifications:

• Valid state nursing license required.
• Proven experience in LTC or SNF settings preferred.
• Demonstrate strong assessment and critical-thinking skills.
• Exhibit excellent communication and interpersonal skills.
• Comply with current vaccination requirements or valid exemptions.
• Ability to wear a photo badge and present valid identification at all times.
