Ennoble Care is seeking a full-time Hospice Registered Nurse Case Manager to work in the Camden / Gloucester County, NJ region. The RN Case Manager will join the interdisciplinary team, conducting skilled nursing assessments, planning and care to maximize comfort for patients and caregivers.
Responsibilities include caseload management, coordinated care with physicians and Hospice staff, and timely EMR documentation.
