Case Manager Registered Nurse - Field; Middlesex

The Case Manager Registered Nurse will develop proactive care plans to enhance member outcomes and overall wellness. This role is ideal for experienced nurses who are passionate about serving dual eligible populations and addressing complex health and social challenges.

  Full-time, Standard business hours Monday-Friday 8:00am-5:00pm EST

  : 25-50% travel required

• Develop proactive plans of care to enhance member outcomes.
• Conduct evaluations of member needs using clinical tools and data review.
• Apply clinical judgment to reduce risk factors and address health indicators.
• Conduct assessments considering information from various sources.
• Use a holistic approach to assess the need for referrals to clinical resources.
• Collaborate with supervisors and stakeholders to overcome barriers.
• Present cases at interdisciplinary case conferences.
• Utilize case management processes in compliance with regulations.
• Employ motivational interviewing skills for member engagement.
• Coordinate all case management activities to evaluate medical needs.

• Associate's Degree required, Bachelor's degree preferred

• Minimum 3+ years of clinical practice experience
• Minimum 2+ years in Care Management, Discharge Planning, and/or Home Health Care Coordination preferred

• Active and unrestricted RN licensure in the state of NJ required
• Certified Case Manager preferred
• NJC Certification preferred but not required
