- Manage a dedicated patient panel, delivering ongoing in‑home primary care
- Care for members with chronic illnesses and advanced medical needs
- Participate in a shared on‑call rotation to ensure continuous support
- Provide comprehensive primary and supportive care directly in members' homes
- Provide longitudinal primary care for members with complex and chronic conditions
- Focus on continuity, quality, and outcomes
- Participate in ongoing care planning with a high degree of clinical autonomy
- Build lasting patient relationships and manage medically complex cases throughout the continuum of care
- Located in or near Mercer, Middlesex or Somerset County, New Jersey, and able to travel locally for home visits
- Active and unencumbered NJ Registered Nurse license
- Certified Advanced Practice Nurse Practitioner (APN)
- Master's degree or post-master's certificate in advanced clinical practice
- Minimum of 1 year of NP experience in a clinical setting
- Active DEA and CDS registration
- Current Basic Life Support (BLS) certification
- Experience in home care or house call settings preferred, but not required
Demonstrates expertise in providing in‑home primary care for patients with chronic illnesses, ensuring continuity and quality of care. Holds advanced clinical qualifications and actively engages in care planning and patient relationship management.
