Position Summary

Provides community outreach, intensive case management, case coordination, assessments, and other specialized activities and functions.

Duties and Responsibilities
  • Provides intensive case management and outreach support
  • Performs specialized activities and functions specific to the program.
  • Coordinates and facilitates community activities
  • Completes documentation and other reporting requirements.
  • Meets regularly with supervisor and participates in other continuing professional development opportunities.
  • Consult with collaborating partners at Camden County Metro police, Camden County Superior Court, Camden City Public Schools, CMO, DCPP, and other community partners.
  • Other job‑related duties as needed to support the program
  • Provides intensive case management and intervention services.
  • Conduct biopsychosocial and substance use assessments to assist families with designing the level of intervention needed to resolve the immediate concerns.
  • Assist families with designing treatment plans and establishing service goals.
  • Completes promptly and accurately all statistical and other reports required for supervisory and administrative purposes and records case material promptly.
  • Link families to community resources that meet their needs within the least restrictive environment & provide follow‑up contacts to ensure services are delivered as planned.
  • Meets weekly with supervisor to report the progress of families.
  • Provides crisis intervention, counseling, advocacy, case management, referral, aftercare plan, and coordination of services.
  • Develops/promotes relationships with other social service agencies and represents the program and agency in the community.
  • Participates in community planning and/or advocacy efforts related to issues designated as relevant to the program's objectives.
  • Participates/conducts professional seminars, workshops, etc., or in‑service training programs. Provides consultation to agencies and organizations in areas of program relevance.
  • Work closely with Camden County Metro to establish & maintain protocols for communication & referral.
  • Provide resource information to educate families on navigating the systems that interface with their juvenile.
  • Attend continuing education training specific to the program.
  • Aid other programs as needed.
  • Responsible for data collection as instructed by Camden County Metro.
  • Provide support to Camden County Metro as needed to support Camden City residents.
  • Other related duties as required.
Requirements
  • Bachelor's degree in related field.
  • At least 3 years of case management experience in social services or mental health.
  • At least 3 years experience in community outreach.
  • Knowledge and experience relative to population served.
