The RN Registered Nurse Admissions and Discharges is primarily responsible for the following job duties:
Assists with ensuring rooms are ready for new admissions
Greet new patients warmly and orient them to their new environment
Complete admission assessments and documentation, including initiation of the baseline care plans
Communicate patient status with interdisciplinary team
Complete medication reconciliation and order entry
Obtain consents if needed
Assist with patient and caregiver education
Ensure discharge orders are accurate
Complete discharge documentation
Complete comprehensive discharge education on post-discharge instructions, including wound care, or new medications.
Coordinate post discharge transition of care appointments
Complete post discharge follow up calls and documentation
Help identify patients at high risk for rehospitalization and implement early interventions
Work collaboratively with interdisciplinary team members to ensure smooth transition
Complete admission paperwork with the resident/resident representative (MA 401)
Assist with other duties as assigned
Must be a graduate of an accredited school of nursing, college, or university; BSN preferred. Current Registered Nurse licensure by the State Board of Nursing Benefits:
