Role And Responsibilities
This position is for a Geriatric Nurse Practitioner providing primary care services in a clinic setting. Responsibilities include managing chronic conditions such as dementia, arthritis, diabetes, and cardiovascular diseases, conducting comprehensive physical exams and cognitive screenings, overseeing medication management, and coordinating patient transitions between hospitals, rehab centers, and home care. The clinician will work during day shifts in an on-site clinic, with a patient load of approximately 10 patients per day.
QualificationsAnd Skills
Candidates must be board certified with prescriptive authority, and have experience in geriatric primary care. Support staff includes MAs and RNs to assist in clinical duties. No call coverage is required.
