• Complete in‑home assessments to evaluate patient condition and environment.
• Develop personalized care plans aligned with medical provider directives.
• Perform wound care, medication administration, injections, IV therapy, and monitoring.
• Teach patients and caregivers disease‑management strategies and safety protocols.
• Maintain detailed, timely documentation for all visits.
• Communicate with providers, therapists, and case managers to coordinate care.
• Prioritize caseloads efficiently and manage daily travel routes.
• Support continuity of care and safe transitions.

• Active Kentucky RN license (or compact eligibility).
• Current BLS certification.
• Minimum 1 year of Home Health nursing experience.
• Reliable transportation for patient visits.
