Responsibilities
  • Conduct occupational therapy evaluations and develop individualized treatment plans for residents
  • Provide and supervise OT services to improve activities of daily living, mobility, and cognition as appropriate
  • Implement therapy sessions, monitor progress, and adjust plans based on resident response
  • Document progress in the resident’s medical record and communicate findings with the interdisciplinary team
  • Educate residents and families on therapy goals, home exercise programs, and safety strategies
  • Ensure care is delivered in compliance with applicable regulations and facility policies
Requirements
  • Active Occupational Therapist license in Connecticut
  • Degree from an accredited OT program
  • Experience in geriatric or long-term care settings preferred
  • Ability to work a 5x8 day schedule and participate in travel-based assignments
  • Strong communication, documentation, and collaboration skills
