OverviewAge Right Clinical Services (ACS) was created to meet the ever-increasing medical needs of older adults and geriatrics. Founded by long-term care providers for long-term care providers, ACS is the first and only clinical group of its kind. As a clinician with Age Right Clinical Services, you will work with a team of dedicated physicians, APPs, nurse practitioners, and physician associates from a variety of specialties to deliver compassionate, holistic care to our patients.
This is an opportunity for an experienced Family, Adult, or Geriatric Nurse Practitioner or Physician Associate who desires to work in a clinic setting within a senior living community, enjoys autonomy, and wishes to exercise their clinical skill set in an evolving environment.

Monday-Friday, 8 a-5 p with no on-call or after-hours requirements.
Responsibilities- Conduct clinical interventional and episodic direct care visits, and appropriate re-evaluation of care visits.
- Diagnose and treat common acute illness and injuries.
- Order and interpret diagnostic tests.
- Develop and implement treatment plans.
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care.
- Perform minor surgical procedures as needed (repair lacerations, I & D, etc.).
- Review medications, orders, and diagnostic tests, and respond to clinical abnormalities as allowed and/or requested by collaborating physicians.
- Engage in advanced care planning conversations.
- Conduct Annual Wellness Visits on all senior living residents, as applicable.
- Coordinate care with other disciplines such as therapists, specialists, and social workers.
- Current, unrestricted Oregon state license, or ability to obtain an Oregon state NP or PA license and current board certification in your field.
- Master's Degree in Nursing or Physician Assistant Studies.
- Experience working with older adults in a clinic, medical office, or urgent care setting preferred.
- Proficiency in medical documentation and coding.
- Current DEA certificate.
