Location: Cañon City
- Coordinate and assist with accurate and timely completion of interdisciplinary MDS Assessments, CAAs, and Care Plans following CMS RAI Manual Regulations.
- Report patient condition changes to the Director of Nursing.
- Educate care staff on updates to CMS regulations and documentation standards.
- Review and validate assessment reports and ensure proper submission into the CMS system.
- Monitor CMS reports for late assessments and develop processes to prevent delays.
- Participate in interdisciplinary meetings and review care plans.
- Perform staff nurse duties as needed and provide excellent patient service.
- Assist with patient evacuations and demonstrate dependable attendance.
- Communicate effectively within a team and adhere to safety, confidentiality, and compliance policies.
RN MDS Coordinator

- Associate’s or bachelor’s degree in nursing from an accredited institution.
- Active RN license in good standing.
- Minimum of two years’ nursing experience, preferably in geriatrics.
- CPR certification (must be current).
- Advanced knowledge of practice standards, regulations, and procedures.
- Ability to make independent decisions and demonstrate professionalism and integrity.
- Strong communication, reasoning, and judgment skills.
- Physical ability to perform required tasks, including mobility and lifting.
