  • Coordinate and assist with accurate and timely completion and submission of interdisciplinary MDS Assessments, CAAs, and Care Plans in compliance with CMS RAI Manual Regulations.
  • Review assessments, validate reports, and ensure assessments are entered into the CMS QIES system before billing.
  • Report patient condition changes to the Director of Nursing and review the Interdisciplinary Care Plan.
  • Provide education to care staff on CMS regulations and documentation updates.
  • Participate in interdisciplinary meetings and review CMS Reports to prevent delays and errors.
  • Perform staff nurse functions as needed and exhibit excellent customer service.
  • Assist with patient evacuation and ensure safety protocols.
Role:

RN MDS Coordinator

Responsibilities

Qualifications & Skills
  • Associate’s or bachelor’s degree in nursing from an accredited institution.
  • Active RN license in good standing.
  • Minimum of two years’ nursing experience, preferably in geriatrics.
  • CPR certification (obtained upon hire or during orientation).
  • Advanced knowledge of post-acute care practices, laws, and regulations.
  • Ability to make independent decisions, maintain confidentiality, and adhere to policies.
  • Strong communication, interpersonal, and organizational skills.
  • Ability to perform physical activities such as lifting and moving patients.
  • Proficiency in English, with good reasoning and judgment abilities.
