Transitions Hospice Community Liaison
Sales professional with hospice, home health, or healthcare experience is a must. Ideal candidate will have strong relationships with senior living communities, MD offices, hospitals, etc. Projected territory:
Cobb Cherokee Pickens Counties. Candidate must live in this area. Licensed Respiratory Therapists open to apply.
A community liaison serves as a first impression for Transitions Hospice with referral sources, patients, families, and the community. A community liaison develops and markets to physicians, community health facilities, office-based practices, private practices, group practices, hospitals, nursing homes, assisted living facilities, and other accounts identified as potential referral sources. A community liaison is provided with an account territory list to manage and works to add new accounts to their list by developing healthy relationships with potential referral sources.
A community liaison is responsible for developing customer relationships, educating patients, and families regarding our services. They work with the Transitions Hospice Intake department to coordinate evaluations, collect necessary clinical documentation, and other tasks related to the procurement, processing and admission of referrals. A candidate must be able to effectively communicate with team members and referral sources from a diverse background in order to provide updates, information, and education related to referrals.
- BA or BS degree preferred.
- In lieu of education, 3 years prior healthcare sales experience.
- Skills or Special Training in Public Speaking, Presentation Skills and Organizational Skills.
- Knowledge of (or ability to learn) Medicare Hospice Program.
- Excellent sales skills with a proven track record of success over a multi-year period in healthcare.
- Possesses a history of building and influencing client relationships, as well as setting and achieving long and short-term goals.
- Holds a current drivers license, car insurance, and positive driving record.
- Hospital, home health, or hospice sales experience is a MUST.
- Establish relationships with physicians, hospitals, and care facilities that produce patient referrals.
- Works with leadership to develop monthly, quarterly and yearly business plans and goals.
- Coordinates community relations, marketing, and business development activities to generate positive name recognition.
- Delivers presentations to healthcare professionals as well as the general public at health fairs and various group settings.
- Identifies and develops new referral sources.
- Meets and exceeds established referral and admission goals. Functions independently, makes sound decisions, solves problems as they arise, and utilizes necessary resources.
- Works to assure mutual goal setting and achievement standards within the marketing team.
- Promotes the hospice mission and vision within the community.
- Projects a professional attitude/appearance.
- Performs other duties as assigned by their direct supervisor.
