• Provide physical therapy services to patients in a skilled nursing setting, including evaluations, treatments, and progress reports
• Develop and implement individualized treatment plans in collaboration with patients, families, and healthcare team members
• Conduct thorough assessments, identify goals, and set realistic treatment objectives
• Utilize a variety of therapeutic techniques and modalities to achieve optimal outcomes
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Maintain accurate and up-to-date records of patient progress and treatment plans

• Current Physical Therapy license in the state of Georgia
• Experience as a Physical Therapist in a skilled nursing setting
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with minimal supervision
