• Provide direct patient care, including vital sign checks, wound care, and medication administration
• Develop and implement individualized care plans in collaboration with the healthcare team
• Conduct assessments and document patient information accurately and thoroughly
• Participate in interprofessional meetings and communicate effectively with colleagues and families
• Maintain a clean and organized work environment, adhering to infection control protocols
• Collaborate with other healthcare professionals to ensure seamless care transitions

• Current LPN license in the state of Georgia
• Previous experience in a healthcare setting, preferably in a long‑term care facility
• Ability to work independently and as part of a team
• Effective communication and interpersonal skills
• Basic computer skills and familiarity with electronic medical records
