Presidential Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Canton, GA. Responsibilities- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Develop and follow individualized patient care plans in coordination with supervising RNs and physicians
- Observe and document patient conditions, reporting any changes to the appropriate healthcare team members
- Assist patients with activities of daily living (ADLs) and personal hygiene as needed
- Educate patients and family members on care procedures, medications, and health management
- Maintain accurate and timely clinical documentation and patient records
- Ensure compliance with all state, federal, and agency healthcare regulations and standards
- Active Licensed Practical Nurse (LPN) license in the state of Georgia
- Previous home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of nursing procedures, medical terminology, and patient care best practices
- Excellent communication and interpersonal skills with patients, families, and care teams
- Reliable transportation and valid driver's license for home visit travel in the Canton, GA area
- CPR/BLS certification required or ability to obtain upon hire
- Compassionate, patient, and detail-oriented with a commitment to quality care
