Family Medicine Illinois

Carle Health is seeking a Family Medicine Advanced Practice Provider to join our growing Primary Care team in Canton, Illinois.

Position Details Include:

• Join an established practice with two Primary Care Physicians and dedicated support staff
• Position will be based out of our Carle Health Canton outpatient clinic
• Flexible work schedule
• Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination
• Carle Health is a not-for-profit organization with public service loan forgiveness eligibility

• Master's degree in Nursing or Physician Assistant
• Experience in Women’s Preventative Health required

• Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure

Canton is a diverse community of 15,000 located 45 minutes from Peoria, a multi-cultural, midsized Midwestern town located on the Illinois River 2 1/2 hours from Chicago and St. Louis. The compensation range for this position is $56.37 - $73.29/hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
