Weekend Baylor Registered Nurse (RN) – Home Health

Our Weekend Baylor RNs provide one-on-one skilled nursing care in the home or where they reside, helping patients maintain independence while receiving high-quality, personalized care. This role offers autonomy, strong team support, and meaningful patient relationships.
Schedule (Full-Time, Exempt)
- Saturday & Sunday: 7:00 AM – 8:00 PM
- Monday: 4 hours for follow-ups and documentation
Key Responsibilities
- Case Management: Provide direct care and comprehensive case management for assigned patients. This includes conducting Start of Care (SOC), Resumption of Care (ROC), and Recertification visits.
- Clinical Documentation: Ensure all associated OASIS documentation is accurate, timely, and compliant with regulatory standards.
- Plan of Care Development: Take ownership of developing individualized plans of care, as well as the oversight and administration of all prescribed medications and treatments.
- Care Coordination: Maintain and update the schedules of all disciplines (PT, OT, etc.) involved in the patient's home care to ensure seamless delivery of services.
- Patient Advocacy: Monitor patient progress, adjust care plans as needed, and educate patients and caregivers on health management and self-care.
- Leadership: Supervise and support Home Health Aides and LPNs to ensure high-quality care delivery.
Qualifications
- Active RN license
- BSN + 1 year experience OR ASN/Diploma + 2 years experience
- Current CPR certification
- Valid driver's license and reliable transportation
Preferred
- Home health experience
- Behavioral or mental health experience
Physical Demands & Work Environment
- Frequent standing, walking, sitting, and use of hands
- Regular bending, kneeling, and reaching
- Occasional lifting and repositioning of patients
- Ability to lift/push up to and over 25 lbs as needed
- Exposure to clinical environments and infectious materials
