About This Role

The Care Manager – Nursing Field (RN CM) ensures that all members receive timely care management across the continuum, including transitions of care, care coordination, and complex case management. This role is ideal for experienced nurses with strong clinical knowledge and critical thinking skills.

• Perform telephonic member outreach and/or face-to-face encounters using motivational interviewing skills.
• Conduct departmental assessments to identify member needs, health goals, and barriers to wellness.
• Develop and implement member-specific plans of care in collaboration with members and providers.
• Complete documentation according to departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care post-hospital discharge.
• Collaborate with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.

• Bachelor’s degree required, preferably in nursing.

• Minimum of 5 years’ relevant clinical experience required.
• Experience in home care or case management preferred.

• Current unrestricted RN license in state of residence.
• National certification in Case Management desirable.
